FBIN Wellbeing Program - Questions & Answers

The Fortune Brands Wellbeing Program is new for 2025, and a component of the new program
includes a cash reward incentive for associates. The program is designed to help you take
charge of your health with preventive and annual screenings.

We’ve partnered with Livez for you to simply complete your health screenings virtually and to
assist you with appointment scheduling for screenings that require a visit to a physician to earn
up to $250 in rewards.

Open to associates only, the cash reward incentive is available for activities completed
from January 1 through November 30, 2025.

Livez is a free preventive health screening app offering personalized care plans, health self-
tests, and professional screening appointments, including an option to easily book your
appointments with your existing in person doctors and providers, or find new ones in-network
and book directly through the Livez app.

All activities follow validated medical guidelines.

To download Livez, visit livez.ai/fbin or search on your app store for “Livez”.

Tier 1: Complete your first self-test ($50).

Tier 2: Finish all available self-tests (additional $75, total $125).

Tier 3: Complete your personalized care plan and schedule provider screenings (additional $125, total $250).

You’ll be notified via email, SMS, or directly in the Livez app when you achieve each milestone.

Good news! If you have already been participating in the Livez app in 2025 and completed any self tests or screenings in 2025, you will receive credit for what you have completed year to date.

Once you receive an email confirming that you’ve completed a program milestone, your reward will be ready to claim.

Process: Your reward will be sent to Alight and added to your Wellbeing Rewards Account. To claim it, visit
fortunebrands.benefitsnow.com and log in. Once you submit your request, your reward will be processed and added to a future paycheck.

Please note: Reimbursements will begin processing in July.

Self-tests are health assessments within the Livez app, covering areas like cardiovascular health, cancer screenings, anxiety, prediabetes, and more.

Screening appointments that cannot be done with the Livez application that need to be done in-person. These appointments can be booked via the Livez app, either with your preferred provider or an in-network provider selected by Livez. If you already have appointments booked directly with your provider you can click “mark as done” to receive credit.

Livez customizes your preventive care plan based on your age, gender, race, lifestyle, and personal and family medical history.

It follows clinically validated guidelines from trusted sources like the CDC, USPSTF, and other U.S. preventive health authorities to recommend the screenings and self-tests most relevant to you.

Yes, your data is strictly protected according to HIPAA and AWS SOC2 standards. Using Livez is confidential and your data will not be disclosed to Fortune Brands, insurance companies, or third parties.

While Livez is available to all full-time Fortune Brands associates, their spouses and dependents aged 18+, the $250 screening incentive is only available for Fortune Brands associates to earn.

No, it is preferred that you use your personal email.

You earn points every time you complete a self-test or a appointment visit through Livez. Points can be redeemed for rewards like entries into raffles for amazing prizes, like Apple devices and more.

The rewards program is open to everyone including associate spouses and dependents. They can earn and redeem points just like you.

To see your current points and available rewards, go to the Rewards tab in the app. New raffles and gift cards are added regularly—so check back often!
